We don't need a new ExpectedOutputs/ convention. Expected outputs are
just another form of test input.

Rename AlternateExtensive to Extensive.
In 2013, five years ago, we have switched to AlternateExtensive
diagnostics by default, and Extensive was available under unused,
undocumented flag.
This change remove the flag, renames the Alternate
diagnostic to Extensive (as it's no longer Alternate), and ports the
test.

Add PostAllocatorCall program point to represent the moment in the analysis
between the operator new() call and the constructor call. Pointer cast from
"void *" to the correct object pointer type has already happened by this point.
The new program point, unlike the previously used PostImplicitCall, contains a
reference to the new-expression, which allows adding path diagnostics over it.

It was written as "Memory Error" in most places and as "Memory error" in a few
other places, however it is the latter that is more consistent with
other categories (such as "Logic error").

This patch adds hashes to the plist and html output to be able to identfy bugs
for suppressing false positives or diff results against a baseline. This hash
aims to be resilient for code evolution and is usable to identify bugs in two
different snapshots of the same software. One missing piece however is a
permanent unique identifier of the checker that produces the warning. Once that
issue is resolved, the hashes generated are going to change. Until that point
this feature is marked experimental, but it is suitable for early adoption.

Summary:
-fno-exceptions does not implicitly attach a nothrow specifier to every operator
new. Even in this mode, non-nothrow new must not return a null pointer. Failure
to allocate memory can be signalled by other means, or just by killing the
program. This behaviour is consistent with the compiler - even with
-fno-exceptions, the generated code never tests for null (and would segfault if
the opeator actually happened to return null).

...but don't yet migrate over the existing plist tests. Some of these
would be trivial to migrate; others could use a bit of inspection first.
In any case, though, the new edge algorithm seems to have proven itself,
and we'd like more coverage (and more usage) of it going forwards.

As mentioned in the previous commit message, the use-after-free and
double-free warnings for 'delete' are worth enabling even while the
leak warnings still have false positives.
