Remove all file headers 
The MIT license doesn't require that all files be prepended with this
licensing or copyright information. Realm confirmed that they're ok with this
change. This will enable some companies to contribute to SwiftLint and the
date & authorship information will remain accessible via git source control.

Aaron McTavish 00b8e79318
Add `max_empty_lines` config to `vertical_whitespace` 
Update `vertical_whitespace` rule to allow configuration of the number
of consecutive empty lines before a violation using `max_empty_lines`.
The default value is still 1 line.
